Description
Spanning from the anterior superior iliac spine to the pubic tubercle, the inguinal ligament is rendered as the rolled inferior margin of the external oblique aponeurosis along the anteroinferior abdominal wall. Superior to it lie the iliopsoas muscles, with psoas major descending from the lumbar vertebrae to join iliacus in the iliac fossa, then passing anterior to the hip joint toward the femur. The pelvic girdle is shown in anterior relation to the lower lumbar spine and sacrum, with the sacroiliac ligaments highlighted posteriorly and the proximal femora seated laterally in the acetabula. Bony landmarks are clear. Clinically, this is the reference line for the groin, separating structures that pass deep to it into the femoral region from the abdominal wall above, and it frames the myopectineal orifice where inguinal and femoral hernias occur. The ligament’s medial attachment near the pubic tubercle provides a fixed point for locating the superficial inguinal ring and for orienting repairs such as open Lichtenstein or laparoscopic TEP and TAPP mesh placement.
