Description

Prominent Kupffer cells line the hepatic sinusoids of a classic liver lobule, positioned within the sinusoidal lumen along the course of mixed blood flowing from the portal triads toward the central vein. At the lobule periphery, portal venules (portal vein branches) and hepatic arterioles enter adjacent to bile ductules, while the sinusoids radiate centrally between plates of hepatocytes toward the terminal hepatic venule (central vein). Spatially, the portal triads sit at the lobular corners, and the central vein occupies the geometric center. Color coding separates inflow vessels from bile outflow pathways. Placement of Kupffer cells directly in the sinusoidal stream explains their outsized impact on hepatic immunobiology and microcirculation. These resident macrophages clear gut-derived bacteria and endotoxin delivered via the portal vein, and they mediate early inflammatory signaling in sepsis, alcoholic hepatitis, and nonalcoholic steatohepatitis. Their sinusoidal location also matters in hemodynamics: Kupffer cell activation can contribute to sinusoidal endothelial dysfunction and increased intrahepatic resistance, a relevant concept when teaching the pathophysiology behind portal hypertension.
