Description
Captured in left lateral profile, the oral commissure (labial commissure) sits at the junction of the upper and lower lips, where the vermilion borders converge and the lip contour turns posteriorly onto the buccal mucosa. Deep to this cutaneous landmark lies the modiolus, a fibromuscular node where orbicularis oris interlaces with buccinator and the perioral elevators and depressors of the mouth angle, including zygomaticus major, risorius, levator anguli oris, and depressor anguli oris. Anterior facial soft tissues, the labial sulcus, and the mandibular contour frame the region, with the highlighted zone emphasizing the commissural complex rather than the dental arches. Clinically, the labial commissure is where form and function collide: minor changes in the modiolus or orbicularis oris tension shift oral competence, speech articulation, and smile symmetry. Facial nerve dysfunction (buccal and marginal mandibular branches) commonly declares itself here as droop or blunting of the mouth angle, while reconstructive planning for cleft-related deformity, trauma, or tumor resection relies on accurate commissure position relative to the philtrum, nasolabial fold, and mandibular border.
