Description

Superior to the ventricles, the ascending aorta and pulmonary trunk emerge and bifurcate toward the pulmonary hila, while the superior vena cava returns venous blood to the right atrium from a right-sided, superior course. Lobation is readable from the front, with the right lung’s superior and middle lobes separated by the horizontal fissure and the left lung showing the cardiac notch and lingula draping over the anterior surface of the left ventricle. Inferiorly, the liver occupies the right upper quadrant beneath the diaphragm, closely related to the heart’s diaphragmatic surface and the inferior vena cava. This anterior view matters when teaching cardiopulmonary topography: the heart’s right atrium and right ventricle sit more anterior than the left-sided chambers, which helps explain why right ventricular injury is a concern in penetrating anterior chest trauma. The spatial relationship between the pulmonary trunk, ascending aorta, and superior vena cava also maps directly to what surgeons and interventionalists navigate during median sternotomy, central venous access, or pulmonary artery catheter placement.
