Description
Running along the lateral aspect of the distal leg and foot, green-highlighted peripheral nerves descend posterior to the lateral malleolus and fan across the dorsolateral and plantar-lateral foot over the tarsus, metatarsus, and calcaneus. Proximally, the course is consistent with the common fibular (peroneal) nerve dividing into superficial and deep fibular branches, while posteriorly the sural nerve tracks toward the lateral border of the foot. Distally, cutaneous branches distribute toward the dorsum and along the plantar surface, approaching the region of the dorsal venous arch and the lateral plantar tissues. Clinically, this lateral view is the one you want when teaching sensory territories and entrapment sites around the ankle. The sural nerve is at risk during lateral hindfoot approaches and small saphenous vein procedures, and it is a frequent generator of postoperative neuritis or painful neuroma when incisions stray posterior to the lateral malleolus. Superficial fibular nerve branches become vulnerable in ankle arthroscopy portal placement and can explain numbness over the dorsum of the foot after inversion injuries.
