Description

Centered on the lumbosacral spine and bony pelvis, the view includes the lumbar vertebrae tapering to the sacrum, the iliac wings with acetabula laterally, and the proximal femora descending inferiorly toward the hip joints. Superimposed in an x-ray style, green lymphatic vessels track along the pelvic brim and medial thigh, converging into clusters of inguinal lymph nodes inferior to the inguinal ligament and iliac nodes positioned along the external and internal iliac chains. Medially, channels from the perineal region and proximal lower limb course superiorly toward the pelvis, while more lateral collectors follow the femoral triangle before turning deep. Pelvic and inguinal lymphatic anatomy matters because these nodal basins sit directly in the pathway of lower-limb infection and malignancy, and they are routinely interrogated on cross-sectional imaging and during oncologic staging. Metastatic spread from penile and scrotal carcinomas often reaches the superficial inguinal nodes first, whereas testicular tumors classically drain to the para-aortic (lumbar) nodes, a distinction rooted in embryologic descent and a common source of exam errors. Surgeons also rely on these relationships when planning groin dissections, sentinel node biopsy, or avoiding lymphatic disruption that can lead to postoperative lymphedema and lymphocele.
