Description
Anteriorly in the male larynx, the median cricothyroid ligament (median cricothyroid membrane, ligamentum cricothyreoideum medianum) spans the midline gap between the inferior border of the thyroid cartilage and the superior border of the cricoid arch. Superior to it sits the thyroid lamina with its laryngeal prominence, while inferiorly the first tracheal rings continue the airway column. Laterally, the ligament blends into the cricothyroid muscles and the lateral cricothyroid membrane, framing the anterior subglottic space just below the level of the true vocal folds. A small target. Clinically, this midline structure defines the access point for cricothyrotomy, where correct placement stays between the cricoid and thyroid cartilages to avoid the cricothyroid muscle bellies laterally and to minimize hemorrhage from superior thyroid vessels. The male thyroid cartilage often presents a sharper laryngeal prominence and a narrower anterior neck angle, which changes palpation landmarks and can affect the perceived width of the cricothyroid interval during emergency airway access. Because the vocal folds lie superior to this membrane, incision too high risks laryngeal injury, while a low incision approaches the cricoid and upper tracheal rings with higher risk of subglottic stenosis.
