Description
Centered on the muscularis externa of the small intestine, the myenteric (Auerbach) plexus runs in the connective tissue plane between the inner circular and outer longitudinal smooth muscle layers, sending fine nerve fibers circumferentially and longitudinally along the bowel wall. Internal to it, the mucosa forms villi over the lamina propria and muscularis mucosae, while the submucosa lies deeper than the mucosa and carries submucosal vessels, lymphatics, and the submucosal (Meissner) plexus. Externally, a thin serosa (visceral peritoneum) covers the intestine. Orientation is clear. Deep, mucosa. Superficial, serosa. Enteric innervation matters because peristalsis is not just muscle, it is wiring. The myenteric plexus coordinates propagating contractions and sphincter relaxation from duodenum through jejunum to ileum, and its injury or congenital absence underlies classic motility disorders such as Hirschsprung disease (aganglionic distal bowel with proximal dilation). Surgeons also care about this plane: full-thickness bowel injury and ischemia disrupt myenteric ganglia, while mucosa-sparing techniques or submucosal dissection aim to preserve neuromuscular function.
