Description
Centered on the anterior thoracic wall, the precordium is framed by the manubrium and body of the sternum superiorly and by the xiphoid process and adjacent costal cartilages inferiorly, with the epigastric region beginning just below the xiphisternal junction. Laterally, the parasternal lines track along the costal cartilages where the cardiac silhouette most closely approaches the chest wall, and the suprasternal (jugular) notch marks the superior border of the manubrium in the midline. A blue highlight in the upper anterior chest corresponds anatomically to the thymus in the superior mediastinum, lying posterior to the manubrium and anterior to the great vessels. Clear surface anatomy. Clinically, the precordium is the working map for cardiac examination and interventions: palpation of the apical impulse near the left 5th intercostal space at the midclavicular line, auscultation points for aortic and pulmonic valves along the 2nd intercostal spaces, and placement of precordial ECG leads V1 to V6 all depend on accurate sternal and costal landmarks. The juxtaposition of the thymus with the sternal notch and manubrium also matters in anterior mediastinal assessment, where thymic hyperplasia or thymoma (classically associated with myasthenia gravis) informs imaging interpretation and surgical planning. Median sternotomy and upper partial sternotomy approaches both navigate this corridor.
