Description
Arising as a series of digitations from the lateral surfaces of the upper ribs, the serratus anterior fans posteriorly to insert along the anterior aspect of the medial border of the scapula, wrapping the thoracic wall like a broad sheet. The scapula and clavicle are rendered in relation to the proximal humerus, with the glenohumeral joint centered laterally and the cervical to thoracic vertebral column providing a clear posterior midline reference. Overlying and adjacent scapular muscles are suggested around the spine of the scapula and the rotator cuff footprint, helping orient the serratus to the costal surface of the scapula rather than the posterior blade. Fiber direction is easy to follow. Serratus anterior anatomy matters when you want to explain scapulothoracic motion, because its contraction protracts and upwardly rotates the scapula while maintaining close apposition to the rib cage during forward elevation of the arm. Weakness or denervation, classically from long thoracic nerve injury after axillary surgery, chest tube placement, or traction, produces medial scapular winging and loss of smooth overhead reach. This view also supports teaching safe injection and incision planning around the lateral thoracic wall by clarifying where the muscle overlies the ribs and intercostal spaces.
