Description
Arising from the common fibular (peroneal) nerve near the fibular neck, the superficial fibular nerve descends in the lateral compartment of the leg deep to fibularis longus and brevis, then becomes subcutaneous in the distal third of the leg. Distally it divides into the medial and intermediate dorsal cutaneous nerves, which course anterior to the ankle and spread across the dorsum of the foot toward the dorsal digital branches to the toes. Bony landmarks include the distal tibia and fibula with the talus at the ankle mortise, continuing into the tarsals, metatarsals, and phalanges, with the nerve branches running superficial to these osseous contours and alongside the superficial venous network shown in blue. Small vessels and cutaneous nerve twigs interweave. It is busy anatomy. Clinically, this distribution explains the classic sensory territory on the dorsum of the foot and anterolateral leg, with relative sparing of the first dorsal web space (deep fibular nerve) and the lateral margin of the foot (sural nerve). The point where the superficial fibular nerve pierces the deep crural fascia is a frequent site of entrapment, producing exertional pain or paresthesia over the dorsal foot, and it is vulnerable during anterolateral ankle arthroscopy portals and transverse incisions across the anterior ankle. Distal branching patterns also matter during dorsal foot approaches to the metatarsals, where iatrogenic neuroma can follow careless dissection. Ideal for lower-limb anatomy teaching in podiatry, physiotherapy, and medical gross anatomy labs, as well as for figure plates in texts covering ankle arthroscopy, fibular fracture exposure, and regional anesthesia or neurologic exam of the fibular nerve.
