Superior Vena Cava

Description

Anterior to the right pulmonary artery and lateral to the ascending aorta, the superior vena cava descends along the right superior mediastinum to enter the right atrium at its superolateral margin. Superiorly, the right and left brachiocephalic veins converge behind the manubrium to form the vessel, with the right brachiocephalic vein taking a short, near-vertical course and the left crossing obliquely from left to right. An x-ray style transparency logically permits partial visualization of adjacent cardiac contours, including the right atrium, superior aspect of the right ventricle, and the proximal great vessels that frame the caval entry point. Color separation emphasizes luminal continuity and caliber change from brachiocephalic confluence to atrial inflow. For procedural anatomy, this relationship is the point. Central venous catheters, pacemaker leads, and ICD leads traverse the brachiocephalic veins into the superior vena cava and terminate near the cavoatrial junction, where malposition can precipitate arrhythmia or even perforation. SVC syndrome from malignancy or indwelling device thrombosis is also taught with reference to the vessel’s fixed position between the sternum and mediastinal structures, which explains why even modest narrowing can produce striking venous congestion of the head, neck, and upper limbs.
