Centered on the posterior thorax, the paired blue overlays mark the triangle of auscultation on either side of the vertebral column, just medial to the medial border of the scapula and inferior to the scapular spine. The boundaries align with the lateral margin of trapezius medially, the superior border of latissimus dorsi inferiorly, and the medial border of scapula laterally, with rhomboid major forming the muscular floor deep to the superficial fascial plane. With the arms held slightly abducted, the scapulae rotate laterally, expanding this gap and thinning the overlying soft tissue between skin and posterior ribs. Surface anatomy, clearly. Clinicians target the triangle of auscultation because it offers one of the least muscle-covered windows to the posterior lung field, improving transmission of breath sounds from the underlying lower lobe, including the posterior basal segments. On physical exam, subtle crackles from dependent atelectasis or early pneumonia often register here before they do through the thicker paraspinal and scapular musculature, and the location is routinely referenced when teaching patients to “sit forward and hug a pillow” to protract the scapula. For procedural planning, the marked region also helps orient learners to safer posterior thoracic landmarks relative to the scapula and ribs when discussing pleural effusion assessment, even though thoracentesis is typically performed more inferiorly in the midscapular line under ultrasound guidance. Use this posterior, skin-surface rendering in gross anatomy and physical diagnosis courses to teach scapular landmarks, back muscle borders, and proper stethoscope placement for posterior lung auscultation across diverse body habitus and skin tone.
