Arising from the dorsal ramus of C2, the greater occipital nerve (nervus occipitalis major) is traced in lateral profile as it ascends from the upper cervical region toward the scalp. Along its course it passes deep to the inferior obliquus capitis and semispinalis capitis, then pierces the trapezius near the superior nuchal line to become superficial and fan out over the posterior vertex. Adjacent landmarks in this angle commonly include the mastoid process, auricle, and layered neck musculature, with the occipital artery running in close company near the nerve’s emergence. Depth relationships are clear. Clinically, this pathway is the anatomic basis for occipital neuralgia and cervicogenic headache patterns, where entrapment often occurs as the nerve traverses the semispinalis capitis or at its superficial exit through the trapezial aponeurosis.
