Description
Arising from the posterior femoral cutaneous nerve, the inferior cluneal nerves curve around the inferior margin of gluteus maximus and pierce the deep fascia at the gluteal fold to supply skin over the inferior buttock. From a lateral perspective, the greater trochanter and proximal femur sit inferior and lateral to the acetabulum, while the iliac crest and lower lumbar spine lie superior, framing the course of the sacral plexus as it converges toward the greater sciatic foramen. Deep and medial to the superficial cluneal branches, the sciatic nerve descends posterior to the hip joint, separated from the skin territory by gluteus maximus. Small cutaneous branches are the point. Pain and dysesthesia along the gluteal crease are often misattributed to lumbar radiculopathy or piriformis syndrome, and this view helps sort out the differential by anchoring symptoms to the posterior femoral cutaneous nerve and its inferior cluneal branches. Clinically, these nerves are vulnerable where they cross the ischial tuberosity region and exit near the gluteal fold, a common site for entrapment in cyclists, irritation from prolonged sitting, or iatrogenic injury with posterior approaches to the hip and gluteal flap surgery. Relationship to the hip joint matters because posterior hip incisions and retraction can irritate the nearby sacral plexus and sciatic nerve, but the inferior cluneal nerves explain isolated inferior buttock numbness without motor deficit. Use this illustration to support teaching of cutaneous innervation of the buttock in gross anatomy, neuroanatomy, and regional anesthesia modules, and to clarify the nerve of origin when labeling cluneal branches in textbooks or surgical atlases.
