Description
Positioned deep in the posterior upper neck, the obliquus capitis inferior runs from the spinous process of C2 (axis) superolaterally to the transverse process of C1 (atlas), forming the inferior border of the suboccipital triangle. A lateral perspective will also place the occipital bone and the posterior arch of the atlas in close profile, with the muscle lying inferior to the superior nuchal line and posterior to the atlantoaxial joint. Nearby landmarks typically visible in this projection include the rectus capitis posterior major superiorly and the semispinalis capitis and splenius capitis lying more superficial and posterior. Deep to the suboccipital layer, the vertebral artery courses in relation to the atlas before turning medially toward the foramen magnum. Clinically, this angle makes the atlantoaxial rotation complex easier to teach because obliquus capitis inferior is a primary rotator of the atlas on the axis, driving ipsilateral head turning while remaining a purely suboccipital, C1-sparing attachment pattern. It also frames the space where cervicogenic headache and occipital neuralgia are discussed, since the greater occipital nerve (dorsal ramus of C2) emerges inferior to the obliquus capitis inferior and can be irritated by muscular hypertonicity or entrapment in this region.
