Description
Seen from a lateral perspective, the cerebral hemisphere dominates the field, with the frontal lobe forming the anterior pole and the temporal lobe lying inferior and slightly posterior along the lateral sulcus. Along the inferoanterior margin, the orbital gyri (gyri orbitales) and intervening orbital sulci are positioned on the orbital surface of the frontal lobe, immediately superior to the anterior cranial fossa and medial to the frontal pole when you infer the ventral contour from the side. Posterior and inferior to the cerebrum, the cerebellar hemisphere is partially color-coded, separated from the occipital lobe by the transverse fissure, while the brainstem descends inferiorly as the midbrain transitions to pons and medulla. Orbital cortex is where anatomy meets behavior and operative risk. Lesions affecting the orbitofrontal region, whether due to traumatic frontal contusions from an anterior cranial fossa impact or a meningioma arising near the planum sphenoidale, often present with disinhibition, impaired decision-making, and altered social conduct even when primary motor and language cortices appear intact. Surgeons approaching the anterior skull base or repairing orbital roof fractures respect this inferior frontal surface because it sits just above the paranasal sinuses and cribriform plate, and postoperative anosmia or CSF leak can coexist with orbitofrontal cognitive change. Small gyri matter here. A lot.
