Description
Running obliquely along the posterolateral thoracic wall, the serratus posterior inferior appears as a thin, quadrilateral sheet arising from the spinous processes of the lower thoracic and upper lumbar vertebrae and passing laterally and superiorly toward the inferior ribs. From this lateral perspective, the muscle lies posterior to the rib cage and superficial to the thoracolumbar fascia, with its serrated slips attaching near the angles of ribs 9 to 12. The thoracic and lumbar vertebral curves, intervertebral discs, and the posterior arcs of the ribs provide clear bony landmarks, while the superior margin of the pelvis sits inferior to the muscle field. Clinically, this is a useful orientation for sorting out posterior accessory respiratory musculature from the deeper intrinsic back muscles and the posterolateral abdominal wall. Confusion at the lower rib angles is common in teaching and in procedural planning, because pain generators in this region range from costovertebral and costotransverse joint dysfunction to lower rib fractures and intercostal nerve irritation. Palpation and ultrasound guided interventions at ribs 9 to 12, including intercostal nerve blocks and lower thoracic paravertebral approaches, benefit from knowing where the serratus posterior inferior overlies the rib angles and blends with the thoracolumbar fascia.
