Description
Oriented in a lateral perspective of the male lower leg and foot, the sural nerve is traced superficially along the posterolateral calf, descending from the region between the heads of gastrocnemius toward the lateral malleolus. Distally, it courses posterior to the fibula and along the lateral border of the calcaneus before continuing as the lateral dorsal cutaneous nerve to the dorsolateral foot and lateral digits. The distal tibia and fibula, talus, calcaneus, and the tarsals, metatarsals, and phalanges provide firm landmarks, while accompanying vessels (rendered in blue) run in parallel along the same corridor. Clinically, this lateral track matters because the sural nerve is a frequent donor for nerve grafting and is also at risk during procedures around the posterolateral ankle, including Achilles tendon repair, peroneal tendon surgery, and lateral malleolar incisions. Entrapment or traction near the lateral malleolus can produce burning dysesthesia over the lateral foot, a pattern that helps distinguish sural neuropathy from L5 or S1 radiculopathy and from common fibular (peroneal) nerve lesions at the fibular neck. For regional anesthesia, the illustration supports accurate placement of a sural nerve block posterior and inferior to the lateral malleolus, where the nerve remains superficial.
