The proximal, middle, and distal phalanges stack from proximal to distal, with the interphalangeal joints forming the toe’s characteristic stepped contour. Plantar to the phalanges lie the flexor tendons and fibrofatty digital pad, while dorsally the extensor expansion thins toward the nail bed. Laterally, the soft tissue envelope blends into the lateral margin of the foot overlying the shaft of the fifth metatarsal. A true lateral view of the little toe matters because deformity is usually assessed in the sagittal plane as much as in the transverse plane. Adductovarus (curly toe) and fifth toe hammering often present as plantarflexion at the proximal interphalangeal joint with varus rotation, creating dorsal corn formation from shoe pressure and plantar callus under the distal phalanx.
