Description
Running from the oblique line of the mandible toward the oral commissure, the depressor anguli oris (triangularis) appears as a superficial, inferolaterally based sheet that narrows superiorly as it approaches the modiolus at the corner of the mouth. Lateral to it, the masseter blankets the ramus of the mandible, its fibers oriented inferiorly toward the mandibular angle, while the body of the mandible and dentition establish the bony framework deep to the perioral soft tissues. Posteriorly, the temporomandibular joint sits between the mandibular condyle and temporal bone, and superior to the mandibular canal region the inner ear anatomy (including the cochlea) is visualized within the petrous temporal bone. Inferiorly, the upper cervical vertebrae align beneath the occiput, giving a clear craniovertebral relationship. Facial expression hinges on fine balances, and the depressor anguli oris is a frequent culprit in a downturned oral commissure with aging, post-stroke synkinesis, or hypertonicity after facial nerve recovery. Injury to the marginal mandibular branch of the facial nerve during submandibular surgery or facelift dissection can denervate this muscle and produce an asymmetric smile, so understanding its superficial position along the mandibular border matters. Masseter prominence and mandibular angle landmarks also intersect clinically with botulinum toxin planning for masseteric hypertrophy or bruxism, where unwanted diffusion can alter lower lip dynamics.
