Seen in lateral profile, the sural region is dominated posteriorly by the gastrocnemius belly, its medial and lateral heads blending inferiorly into the flatter soleus to form the triceps surae contour. Anterolaterally, the tibial crest sits more anterior and medial, while the fibular shaft lies lateral and slightly posterior, creating the subtle bony line that leads distally toward the lateral malleolus. Distal to the muscle bellies, the calcaneal (Achilles) tendon becomes a distinct cord running inferiorly to the posterior calcaneus, separating the muscular calf from the narrower hindfoot silhouette. Clean surface anatomy. This lateral calf view matters because it maps where force transmission and injury commonly declare themselves: pain at the musculotendinous junction of gastrocnemius in a classic tennis leg, focal thickening and tenderness along the Achilles in tendinopathy, or a palpable gap after rupture. The relationship between the superficial posterior compartment and the fibula also anchors discussions of acute compartment syndrome and where swelling most distorts the lateral leg.
