Seen in lateral profile, the lower leg is organized around the fibula as the slender lateral bone running inferiorly from the fibular head and neck toward the lateral malleolus, with the broader tibia positioned anteromedially and forming the medial contour of the shin. Along the lateral compartment, the fibularis (peroneus) longus and brevis muscle bellies occupy the mid to distal leg, their tendons coursing posterior to the lateral malleolus toward the foot. Anteriorly, tibialis anterior and extensor digitorum longus form a more anterolateral plane, while posteriorly the lateral head of gastrocnemius blends with soleus into the calcaneal (Achilles) tendon. Clear surface landmarks. Clinically, this lateral leg view is the one you want when teaching the relationship of the common fibular (peroneal) nerve to the fibular neck, a frequent site of neuropraxia after fibular head fracture, tight casting, or prolonged compression, producing foot drop with weakness of ankle dorsiflexion and eversion. Orthopaedic fixation of distal fibular fractures and syndesmotic injuries also depends on recognizing the subcutaneous course of the distal fibula and the posterior turn of the fibularis tendons behind the lateral malleolus. It also frames the lateral ankle ligament region (anterior talofibular and calcaneofibular ligaments), where inversion sprains concentrate.
