From the lateral aspect of the male hindfoot, the calcaneus (calcaneal bone) dominates the rearfoot, with its posterior tuberosity forming the bony core beneath the heel pad and the lateral wall curving anteriorly toward the calcaneocuboid region. Superior to the calcaneus, the talus is inferred at the subtalar joint, while the fibular malleolus sits proximal and slightly anterior as the lateral ankle landmark. The Achilles (calcaneal) tendon descends posteriorly to insert on the calcaneal tuberosity, and the peroneal tendons course posterior to the lateral malleolus, then run distal and anterior along the lateral calcaneus toward the cuboid. Lateral heel anatomy matters because many pain generators and procedural landmarks cluster in a small area: Achilles tendinopathy and insertional enthesopathy localize to the posterior superior calcaneus, whereas retrocalcaneal bursitis sits deep to the tendon near its insertion. The same surface relationships guide assessment of calcaneal fractures (including displacement and widening at the hindfoot) and orient lateral approaches for subtalar arthrodesis or calcaneal osteotomy, where protecting the sural nerve and peroneal tendons becomes a constant concern.
