Description
Running along the lateral thigh, the iliotibial tract (iliotibial band) appears as a thickened longitudinal condensation of fascia lata extending from the iliac crest and anterolateral pelvis toward the proximal tibia. Proximally it receives fibers from tensor fasciae latae anteriorly and gluteus maximus posteriorly, then courses superficial to vastus lateralis and the posterolateral margin of the quadriceps while lying lateral to the femoral shaft. Distally the tract crosses the lateral femoral epicondyle and continues to its principal insertion at Gerdy’s tubercle on the anterolateral tibia, with the biceps femoris tendon and fibular head lying more posterior and inferior in the same region. A lateral perspective clarifies why the iliotibial tract acts as a tension band across the hip and knee, stabilizing the lateral knee in extension and helping control femoral adduction and internal rotation during single-leg stance. The relationship to the lateral femoral epicondyle is the key teaching point. Friction or compression at this level under repetitive knee flexion and extension underlies iliotibial band syndrome, a common source of lateral knee pain in runners and cyclists, and the same anatomy guides palpation, ultrasound correlation, and targeted injection planning. Sports medicine lectures, gross anatomy labs, and orthopaedic or physiotherapy texts often need a clean depiction of the IT band with tensor fasciae latae and gluteus maximus contributions shown in continuity, and this artwork supports discussions of gait biomechanics, lateral knee pain differentials, and postoperative lateral thigh symptoms after hip surgery.
