Description

Along the lateral (fibular) border of the male foot, the calcaneal tuberosity sits posteriorly and inferiorly, with the lateral malleolus of the fibula positioned superior and slightly anterior as the ankle landmark. Anterior to the calcaneus, the cuboid forms the lateral column of the midfoot, and the base and tuberosity of the fifth metatarsal project laterally as the most palpable bony prominence on the outer margin. Distally, the lateral forefoot tapers toward the fifth metatarsophalangeal region, while the plantar skin thickens under the lateral calcaneus and along the lateral longitudinal arch. A clean lateral profile of the lateral border matters because it captures the landmarks clinicians use to localize injury and to judge alignment in weight-bearing and non weight-bearing assessment. The tuberosity of the fifth metatarsal is the key reference for differentiating an avulsion fracture at the peroneus brevis insertion from a proximal diaphyseal Jones fracture, two entities with different management and healing risk. Peroneal tendon subluxation, calcaneofibular ligament tenderness after inversion sprain, and lateral hindfoot varus also map naturally onto this surface topography.
