Seen in lateral profile, the lower leg is contoured by the posterior bulge of the gastrocnemius and soleus forming the calf, tapering inferiorly into the calcaneal (Achilles) tendon toward the posterior aspect of the ankle. Along the lateral column, the fibula lies posterolateral to the tibia, with the fibular head positioned proximally near the knee and the lateral malleolus defining the distal ankle prominence. The anterior border of the tibia remains more anterior and medial, while the lateral compartment surface landmarks correspond to the course of fibularis longus and brevis as they pass distally toward the foot. Lateral lower-leg views matter because many high-frequency injuries and procedures key off these landmarks, and a side profile clarifies what is anterior versus posterior around the ankle better than a frontal view. Tenderness over the distal fibula and lateral malleolus raises concern for ankle sprain with anterior talofibular ligament involvement, but pain posterior to the malleolus directs attention to fibularis tendon subluxation or tenosynovitis, where swelling can obscure the usual groove and retinacular contour.
