Seen in lateral profile, the adult male hindfoot centers on the calcaneus (calcaneum), with its posterior calcaneal tuberosity forming the palpable heel and its superior surface articulating with the talus at the subtalar joint. The Achilles tendon (tendo calcaneus) descends posterior to the distal tibia and fibula and inserts on the posterior calcaneus, while the plantar aspect of the calcaneus continues anteriorly into the calcaneal tuber and plantar fascia attachment. Lateral bony contours at the calcaneus align anteriorly with the cuboid at the calcaneocuboid joint, framing the lateral column of the foot. Clinically, a true lateral heel view is the cleanest way to teach the Achilles insertional footprint and its relationship to the posterosuperior calcaneus, the region implicated in Haglund deformity and insertional Achilles tendinopathy. It also supports discussions of calcaneal fractures, where the lateral wall blowout and changes in Böhler angle on lateral radiographs correlate with subtalar joint involvement and long-term loss of hindfoot mechanics. Tendon traction matters here. Even a small enthesophyte at the posterior calcaneus can change load transmission during push-off.
