Seen from the lateral aspect, the posterior knee forms the popliteal fossa (kneepit) as a gentle depression between the distal thigh and proximal leg. Superiorly and posteriorly, the biceps femoris tendon defines the lateral boundary, while the lateral head of gastrocnemius and plantaris (when present) rise from the femoral condyle inferior to it; the iliotibial tract courses along the lateral femur to Gerdy’s tubercle, remaining anterior to the fossa. Deep to this surface contour lie the fibular (lateral) collateral ligament and the posterolateral capsule, with the popliteus tendon and muscle angling inferomedially across the posterior tibia. Small saphenous vein and sural cutaneous nerves typically traverse the superficial fascia before joining deeper structures toward the midline. A lateral posterior view matters because the “corner” of the knee is where subtle instability lives. Injury to the posterolateral corner, often involving the fibular collateral ligament, popliteus tendon, and popliteofibular ligament, can produce varus and external rotational laxity and is a common reason an anterior cruciate ligament reconstruction fails if the PLC is missed. Surgeons also use the biceps femoris and fibular head as palpable landmarks when protecting the common fibular nerve as it winds around the neck of the fibula. Know this contour.
