Description
Centered on the thenar eminence of a left male hand in palmar (anterior) orientation, the abductor pollicis brevis is rendered as a flattened triangular muscle lying superficial and radial to the central palm. Its proximal fibers arise from the flexor retinaculum and adjacent carpal region (classically the scaphoid tubercle and trapezium), then sweep distally and laterally toward the thumb, crossing the radial side of the first carpometacarpal and metacarpophalangeal joints. Distally, the tendon blends with the lateral base of the proximal phalanx of the thumb and the dorsal extensor expansion, sitting superficial to deeper thenar components such as opponens pollicis while the metacarpals, phalanges, and carpal bones provide clear bony landmarks beneath. Bony context is explicit. Joint lines read cleanly. Functionally, this is the most teachable surface view for linking thenar contour to median nerve integrity, because the abductor pollicis brevis is commonly tested in clinic by resisted thumb palmar abduction with the hand supinated. Weakness or thenar flattening points you toward median neuropathy, including carpal tunnel syndrome, and helps distinguish it from ulnar lesions that spare the thenar motor branch. The palmar perspective also clarifies why the recurrent branch of the median nerve is surgically vulnerable during thenar or carpal tunnel approaches: it courses superficially into the thenar mass near the distal edge of the flexor retinaculum.
