Description
Seen from a posterior perspective through a semi-transparent male thigh, the femur runs inferiorly from the acetabulum to the knee, with the femoral head and neck positioned superomedial to the greater trochanter and the intertrochanteric region spanning laterally. A fracture focus is marked in red at the proximal femur, at the femoral neck just distal to the femoral head, while the distal shaft and condylar region remain aligned toward the tibiofemoral joint. Medially at the distal femur, the adductor tubercle sits just superior to the medial epicondyle as a clear landmark for orientation, even though the surrounding soft tissues are intentionally minimized. Bone is the priority. Femoral neck fractures carry specific downstream problems because the fracture plane often lies intracapsular, where the ascending cervical (retinacular) branches of the medial circumflex femoral artery can be disrupted as they course along the posteriosuperior neck. That vascular vulnerability explains why displaced Garden III and IV patterns have a higher risk of femoral head osteonecrosis and nonunion than many extracapsular hip fractures, and why urgent reduction, internal fixation, or arthroplasty decisions hinge on displacement and patient factors. Posterior visualization also helps learners map the relationship between the neck, trochanters, and the line of force transmission from pelvis to knee. A common fracture site.
