Description
Posterior scapular anatomy dominates the field, with the scapular spine running transversely to the acromion and dividing the supraspinous fossa superiorly from the infraspinous fossa inferiorly. A fracture line traverses the thin scapular body between the medial border and the lateral border, approaching the inferior angle while remaining posterior to the glenoid cavity and scapular neck. Portions of the adjacent rib cage lie anteromedial to the blade, and the proximal humerus sits lateral and slightly anterior, orienting the scapula within the shoulder girdle. Scapular body fractures usually follow blunt, high-energy mechanisms such as motor vehicle collisions or falls, and the posterior surface is where you appreciate how little cortical thickness separates the infraspinous fossa from the thoracic wall. High-energy trauma is typical. Because the scapula links clavicle to humerus through the glenoid and suspends the upper limb via the periscapular muscles, displacement across the body can alter scapulothoracic motion and contribute to painful shoulder dysfunction even when the glenohumeral joint remains congruent; CT with 3D reconstruction is often used to assess comminution and medialization or angular deformity that might prompt operative fixation. Posterior landmarks also frame the common surgical corridor, using the scapular spine and lateral border to access the body while protecting the suprascapular nerve near the spinoglenoid region.
