Description
Running along the lateral thigh, the iliotibial tract (iliotibial band) appears as a broad condensation of fascia lata extending from the iliac crest to the anterolateral proximal tibia at Gerdy’s tubercle. From a posterior perspective, its proximal fibers blend with the gluteus maximus tendon, then descend superficial to the vastus lateralis while sending septal expansions toward the linea aspera and lateral intermuscular septum. The semi-transparent bony overlay clarifies how the band tracks lateral to the femoral shaft, crosses the level of the lateral femoral epicondyle, and continues toward the tibia just anterior to the fibular head and lateral collateral ligament. Repetitive knee flexion and extension can provoke iliotibial band syndrome, where friction or compression at the lateral femoral epicondyle generates focal lateral knee pain in runners and cyclists, and this posterior-lateral orientation helps you relate symptoms to the band’s course and its tethering to the distal femur. At the hip, impaired gluteus medius and maximus control increases tension transmitted into the tract, a common contributor to lateral hip pain and greater trochanteric pain syndrome, and a frequent target in gait retraining and rehabilitation planning.
