Seen from a posterior perspective in an adult male, the thoracic cage is framed by the vertebral column in the midline and paired ribs sweeping laterally from the costovertebral joints. Deep in each intercostal space, the innermost intercostal muscles form thin, segmented slips spanning between adjacent ribs, positioned internal to the internal intercostals and superficial to the endothoracic fascia. Their fibers generally mirror the internal intercostals, running inferoposteriorly from the rib above toward the rib below, most distinct in the lateral portions of the interspaces where the intercostal neurovascular bundle lies nearby in the costal groove. Posteriorly, this layer matters because it sits in the same anatomic corridor used by pleural procedures. A needle or catheter placed for thoracentesis or a chest tube passed through an intercostal space traverses skin, superficial fascia, external and internal intercostals, then the innermost intercostal layer and endothoracic fascia before reaching parietal pleura. The close relationship to the intercostal vein, artery, and nerve along the inferior border of the rib makes rib-level orientation and staying just superior to the rib below more than a teaching point.
