Posterior to the cerebral hemispheres, the medial surfaces are opened to emphasize deep diencephalic anatomy, with the paired thalami forming a central mass just inferior to the bodies of the lateral ventricles and flanking the third ventricle. Inferior to the thalamus, the hypothalamus occupies the anteroinferior floor and lateral walls of the third ventricle, tapering toward the infundibulum and mammillary region, while the midbrain continues caudally into the pons and medulla oblongata in the midline. Cerebellar hemispheres and vermis sit posteroinferior to the brainstem, their folia distinct from the cerebral gyri and separated from the occipital lobes by the tentorial plane. Landmarks read cleanly. This posterior perspective is a practical way to teach orientation in the ventricular system and the anatomic neighborhoods that drive common clinical findings. Lesions affecting the thalamus or posterior limb of the internal capsule often produce contralateral hemisensory loss or dense hemiparesis, while hypothalamic involvement shifts endocrine and autonomic control, a pattern seen with craniopharyngioma extension, hypothalamic glioma, or compressive effects from third-ventricle masses.
