Description
Positioned on the posterior thoracic wall, the serratus posterior inferior lies deep to latissimus dorsi, spanning from the spinous processes and thoracolumbar fascia of the lower thoracic and upper lumbar region toward the inferior borders of ribs 9 to 12. Its fibers course inferolaterally from the vertebral midline, fanning out to the posterolateral rib angles. Surrounding landmarks in this posterior perspective typically include the thoracic and lumbar vertebrae, posterior ribs, scapulae superiorly, and the iliac crests and posterior pelvis inferiorly. Bilateral symmetry is apparent. Clinically, this muscle is a frequent source of confusion with quadratus lumborum and the lower slips of latissimus dorsi when teaching posterior trunk layers or interpreting focal posterolateral rib pain. Palpation and trigger-point patterns can mimic costovertebral joint irritation or lower rib stress, and its attachments near ribs 11 and 12 place it in the same neighborhood as the floating ribs that are susceptible to fracture and intercostal nerve irritation. The oblique fiber direction makes the muscle a practical landmark when discussing the mechanics of forced expiration and how deep posterior thoracic muscles relate to the thoracolumbar fascia.
