Description
Posterior to the distal thigh and knee, the saphenous nerve is traced as a slender terminal cutaneous branch of the femoral nerve, descending from the femoral triangle and passing deep to sartorius within the adductor canal before becoming superficial near the medial side of the knee. From this vantage, the nerve’s course can be followed along the posteromedial border of the tibia, running inferiorly toward the medial malleolus and medial foot, with the femur superiorly and the tibia and fibula extending distally to the ankle mortise. Bony landmarks, including the femoral condyles, tibial plateau, and the ankle joint surfaces, anchor the nerve’s relationship to the knee and leg skeleton. Saphenous nerve anatomy matters because it is the main sensory nerve to the medial leg and a frequent source of postoperative dysesthesia around the knee and proximal tibia. Injury can occur during medial knee arthroscopy portals, harvest of the great saphenous vein, or procedures around the adductor canal, and entrapment beneath the sartorius fascia can produce medial knee pain that mimics meniscal or pes anserine pathology. Regional anesthesia also targets this pathway, and the illustration helps distinguish a saphenous nerve block (adductor canal block) from sciatic or tibial nerve techniques when medial leg sensation is the goal.
