Description
Running along the posterior midline, the spinous processes form a near-vertical ridge from the upper cervical region into the thoracic and lumbar spine, with C2 (axis) identifiable by its robust bifid spinous process just inferior to the atlas. From this back view, paired laminae converge toward each spinous process while the facet (zygapophyseal) joints sit slightly lateral, stepping down in a regular pattern between adjacent vertebrae. C2’s transverse processes flank the vertebral body laterally, and their transverse foramina mark the cervical level even when the vertebral canal itself is only implied in a posterior perspective. Alignment is the teaching point here. The straightness or lateral deviation of the spinous-process line is a quick surface proxy for coronal-plane curvature, useful when explaining scoliosis screening, postural kyphosis, or the bony landmarks used to count vertebral levels. C2 also matters clinically because instability at the atlanto-axial complex, from trauma, rheumatoid arthritis, or congenital laxity, can threaten the upper cervical spinal cord, and surgeons depend on a clear mental model of the axis when planning C1-C2 fixation or placing C2 pedicle or pars screws.
