Posterior to the stomach, the spleen (lien) sits high in the left hypochondrium beneath the left hemidiaphragm, its convex diaphragmatic surface separated from the posterior abdominal wall by a thin serosa of visceral peritoneum. Peritoneal reflections extend from the splenic hilum as the gastrosplenic ligament to the greater curvature of the stomach and as the splenorenal (lienorenal) ligament toward the left kidney, with the tail of the pancreas typically approaching the hilum within this fold. Inferiorly, the splenic peritoneum continues into the greater omentum via the left portion of the gastrocolic region, relating the spleen to the splenic flexure of the colon while the parietal peritoneum lines the adjacent abdominal wall. A posterior perspective on the splenic peritoneum is the cleanest way to teach how the organ is tethered without obscuring the hilum, and it clarifies where vessels and adjacent viscera travel within peritoneal ligaments. That anatomy matters in splenectomy and distal pancreatectomy, where mobilization often proceeds by dividing the splenorenal and gastrosplenic ligaments while protecting the splenic artery and vein and avoiding traction injury to the pancreatic tail. It also frames why splenic trauma can bleed freely: the capsule and serosa are thin, and the peritoneal attachments transmit force.
