Description

Posterior to the tarsus, the calcaneal (Achilles) tendon descends in the midline from the gastrocnemius and soleus complex to insert on the posterior surface of the calcaneus, just superior to the heel pad. Lateral to the tendon, the fibular (peroneal) tendons course behind the lateral malleolus toward the foot, while medially the tibialis posterior, flexor digitorum longus, and flexor hallucis longus tendons pass posterior to the medial malleolus on their way to the plantar aspect. Superficial landmarks in this region include the small saphenous vein and sural nerve tracking along the posterolateral leg, with the posterior talocrural (ankle) joint line and adjacent subtalar region implied deep to the tendon and malleoli. Clear surface anatomy. Clinically, the posterior ankle is where Achilles tendinopathy, insertional enthesopathy, and acute rupture present, often 2 to 6 cm proximal to the calcaneal insertion where vascularity is limited and degeneration accumulates. The relationship of the tendon to the calcaneus and retrocalcaneal bursa helps differentiate midportion tendinopathy from retrocalcaneal bursitis or a Haglund prominence, and it is exactly the corridor assessed during a Thompson squeeze test or palpation for a tendon gap. Surgical planning also depends on these posterior landmarks, including open or percutaneous Achilles repair and endoscopic approaches to posterior heel pain. Use this posterior view in lower limb anatomy labs, sports medicine teaching on tendon injury patterns, and orthopedic or podiatry publications that need accurate surface correlation for the Achilles tendon, malleoli, and hindfoot.
