Description
Posteriorly oriented anatomy of the male coxal joint is presented with the femoral head seated in the acetabulum of the os coxae. The posterior acetabular rim and adjacent ischium lie superior and medial to the femoral neck, while the greater trochanter projects laterally as a palpable landmark just distal to the joint line. Articular cartilage is absent or thinned over the load-bearing dome, exposing subchondral bone with irregular, erythematous wear consistent with eburnation and marginal osteophyte formation. Degenerative arthritis of the hip most often concentrates at the superolateral acetabulum and corresponding femoral head, and a posterior view helps you correlate those contact zones with the posterior capsular attachment and the region traversed by the short external rotators (piriformis, obturator internus, gemelli). That relationship matters in the classic posterior approach for total hip arthroplasty, where release and later repair of these tendons aims to reduce postoperative posterior dislocation, and where osteophytes along the posterior rim can mechanically limit internal rotation and provoke groin pain. Expect the clinical pattern: reduced hip internal rotation, antalgic gait, and radiographic joint-space narrowing before end-stage bone-on-bone change.
