Diencephalic anatomy dominates this posterior view, with the paired thalami forming the bulk of the dorsal contour on either side of the midline and tapering posteriorly toward the pulvinar region. Superior and slightly posterior to the thalamus, the epithalamus is identifiable by its relationship to the pineal region and habenular area, set just dorsal to the posterior aspect of the third ventricle. Inferior to the thalamus, the hypothalamus occupies the ventral diencephalon, positioned anteroinferior relative to the epithalamus and bordering the walls and floor of the third ventricle as it approaches the mesencephalic junction. Midline neuroanatomy is emphasized. Neighboring posterior midbrain landmarks may be partially visible at the diencephalo-mesencephalic boundary to orient the viewer. These relationships matter when teaching the ventricular system and nearby commissural pathways, because a posterior perspective makes it easier to appreciate how the epithalamus and pineal region sit immediately dorsal to the posterior third ventricle and just superior to the tectal plate. Neurology and neurosurgery trainees often need this orientation when correlating a pineal region mass with obstructive hydrocephalus at the level of the posterior third ventricle or cerebral aqueduct, and when explaining vertical gaze palsy (Parinaud syndrome) from dorsal midbrain compression. Lighting and isolation of deep structures reduce superficial cortical distraction, keeping attention on the thalamic mass, hypothalamic position, and epithalamic roof structures without competing sulcal detail.
