Framed from a posterior standpoint, the female buttocks are defined inferiorly by the gluteal fold (gluteal sulcus or crease), where the skin and superficial fascia tether to the deep fascia at the junction of the buttock and proximal thigh. Superior to the fold lies the convexity created chiefly by gluteus maximus, while inferior and distal to it the contour transitions onto the posterior thigh overlying the hamstring compartment. Medially, the gluteal folds approach the intergluteal cleft, and laterally they fade toward the fascia lata and the line of the iliotibial tract. Surface anatomy matters here. Clinically, the gluteal crease is more than a cosmetic boundary, it functions as a reliable landmark when planning posterior gluteal or proximal thigh incisions, and it helps orient flap design by indicating the inferior margin of gluteus maximus and the typical course of the inferior gluteal perforators.
