Description
Viewed from behind, the female hip (coxa) region is centered on the posterior pelvis and proximal femur, with the iliac crests forming the superior contour and the sacrum and coccyx occupying the midline. Lateral to the sacrum, the posterior superior iliac spines sit as palpable dimples, while the gluteus medius and maximus create the bulk of the posterolateral and posterior buttock, respectively, tapering inferiorly into the gluteal fold. Inferolaterally, the greater trochanter marks the lateral prominence of the proximal femur, and the ischial tuberosity lies deep and inferior near the medial aspect of the proximal thigh. Posterior hip surface anatomy matters because it guides safe injection technique and helps localize deep neurovascular structures that are not visible but clinically exposed in this region. The superolateral quadrant of the buttock corresponds to a safer zone for intramuscular injection in relation to the sciatic nerve, which courses deep to gluteus maximus and emerges inferior to piriformis before descending toward the posterior thigh. Posterior hip pain patterns also map onto these landmarks: greater trochanter tenderness often reflects gluteus medius or minimus tendinopathy rather than isolated trochanteric bursitis, and the posterior approach in total hip arthroplasty relates closely to the short external rotators and posterior capsule. Landmarks matter. Use this posterior view in gross anatomy and surface anatomy teaching to reinforce palpation points (PSIS, iliac crest, greater trochanter) and to anchor discussions of injection sites, posterior hip surgical exposures, and referral patterns around the sciatic nerve.
