Description
Posterior surface anatomy of the male pelvis frames the paired gluteus medius muscles as they fan from the outer (gluteal) surface of the ilium toward the proximal femur. Each muscle sits lateral to the sacrum and posterior iliac crest, spanning between the posterior gluteal line region and the greater trochanter, with the right and left bellies separated medially by the sacral midline and posterior superior iliac spines. Superiorly, lower thoracic vertebrae and the inferior rib margins provide axial context; inferiorly, the proximal femora align with the acetabula to orient the hip joints. Bony landmarks remain legible. Gluteus medius is the primary hip abductor and a key pelvic stabilizer during single-limb stance, so this posterior layout helps correlate muscle fiber direction with gait mechanics. Injury to the superior gluteal nerve (L4–S1), or tendinopathy at the greater trochanteric insertion, produces abductor weakness and the classic Trendelenburg sign, a finding often evaluated after lateral hip pain, total hip arthroplasty, or pelvic trauma. Clinically, the relationship to the posterior iliac crest and trochanter also supports safe planning of gluteal intramuscular injections, which avoid the superomedial quadrant where neurovascular structures are less forgiving. Hip abductor anatomy matters.
