Description

Running along the posterior antebrachium, the extensor compartment is laid out from the lateral supracondylar ridge and lateral epicondyle of the humerus down to the dorsum of the wrist and hand. Laterally, brachioradialis and extensor carpi radialis longus and brevis form the prominent mobile mass toward the radial styloid, while extensor digitorum occupies the central dorsum with extensor digiti minimi positioned just ulnar to it. Medially, extensor carpi ulnaris tracks to the base of the fifth metacarpal, and proximally near the elbow you would expect anconeus posterolateral to the olecranon and the common extensor tendon spanning the lateral epicondyle. This view puts the dorsal tendons in line. Posterior forearm anatomy matters when you teach the logic of wrist and finger extension and when you plan approaches to lateral elbow pain. Many cases of lateral epicondylitis localize to the origin of extensor carpi radialis brevis at the lateral epicondyle, and a posterior view helps clarify why resisted wrist extension reproduces symptoms while grip weakness can follow from pain inhibition rather than true motor deficit. It also frames the compartments relevant to intersection syndrome, where the first dorsal compartment tendons (abductor pollicis longus and extensor pollicis brevis) cross the extensor carpi radialis tendons in the proximal dorsoradial forearm.
