Oriented from a posterior thoracic perspective, the paired lungs sit lateral to the mediastinum with the cardiac silhouette nestled anterior to the left lung and offset slightly to the left of midline. Inferiorly, the domed diaphragm forms the floor of the thoracic cavity, with its central tendon positioned more superior than the peripheral muscular slips that attach along the costal margin. Posterior costodiaphragmatic angles are defined where the parietal pleura reflects from the thoracic wall onto the diaphragm, creating the dependent pleural recess beneath the lower lobes. Bronchial and lobar contours are suggested along the posterior lung surfaces, where the lower lobes occupy most of the field. Posterior anatomy is where pleural fluid announces itself first.
