Description
Posterior lower leg anatomy is presented from the popliteal region to the calcaneus, centered on the superficial posterior compartment. The medial and lateral heads of gastrocnemius form the proximal calf and converge inferiorly with soleus into the calcaneal (Achilles) tendon, which descends centrally to insert on the posterior calcaneus. Along the posterolateral aspect, the small saphenous vein and sural nerve typically track toward the midline as they approach the ankle, while deeper planes would include flexor hallucis longus, flexor digitorum longus, and tibialis posterior lying anterior to soleus, adjacent to the posterior tibia and fibula. Muscle bellies dominate. This posterior perspective matters when you need to teach or explain the mechanics and vulnerabilities of plantarflexion. The gastrocnemius spans both knee and ankle, so knee position changes calf strength and helps clarify why a gastrocnemius strain often occurs at the musculotendinous junction during sprinting, while an Achilles tendon rupture tends to occur 2 to 6 cm proximal to the calcaneal insertion where vascularity is relatively limited. Palpation landmarks in this view also map cleanly onto common clinical findings: tenderness along the Achilles in tendinopathy, swelling in the calf with deep vein thrombosis, and the course of the small saphenous vein used for venous access or bypass planning.
