Description
Seen from behind, the male lower limbs are presented from the gluteal region to the feet, emphasizing the posterior compartment of the leg. Superficially, the gastrocnemius forms the dominant calf contour with its medial head positioned more medial and slightly inferior compared with the lateral head, while the soleus broadens the calf deep and inferior to it toward the calcaneal (Achilles) tendon. Inferiorly, the calcaneal tendon descends centrally to insert on the posterior calcaneus, flanked by the malleolar region at the ankle and continuing distally to the heel and plantar surface. Posterior leg anatomy matters because it is where propulsion and venous return meet clinical risk. The gastrocnemius and soleus converge into the calcaneal tendon, a frequent site of tendinopathy and acute rupture during sprinting or jumping, and a key structure assessed during the Thompson test. Medial calf pain also raises concern for deep vein thrombosis within the deep venous system that accompanies the posterior tibial and fibular (peroneal) arteries, while focal tenderness along the musculotendinous junction often points instead to a gastrocnemius strain.
