Seen from behind, the posterior cervical region (collum) is centered on the midline ligamentum nuchae, which runs from the external occipital protuberance to the spinous processes and makes the C7 vertebra (vertebra prominens) palpable at the cervicothoracic junction. Superficial contours correspond to the trapezius descending from the superior nuchal line and nuchal ligament toward the shoulder girdle, with the posterior border of the sternocleidomastoid forming a subtle lateral boundary. Inferiorly, the neck blends into the upper back over the spinous process of T1, while superiorly it approaches the occipital scalp margin. Palpation landmarks drive much of the clinical value in this view: C7 is commonly used to count cervical levels and to orient surface marking for cervical epidural access, posterior cervical injections, and radiographic localization. Distinguishing C7 from T1 also matters in trauma and spondylotic disease, where malalignment at the cervicothoracic junction can be missed without reliable midline reference points. It is a practical teaching frame for relating skin creases and muscle contours to the deeper cervical spine.
